We have prepared a web application which aims to facilitate the implementation of tasks in the area of public health regarding the creation of health policy programmes (HPP) and reports on their implementation.
This HPP wizard constitutes a free-of-charge web application which allows to log in and register using an e-mail address.
As part of the application, you have at your disposal:
- a HPP project and
- a final report on the implementation of the HPP,
which, as a final result, will create documents in PDF format, in accordance with the Regulation of the Minister of Health of 22 December 2017 on the health policy programme template (…). It is possible to save changes, edit the entered data and return to the previously saved project version at each stage of the project works.
The application also contains ready-to-use templates for health policy programmes (prepared in accordance with the recommendations of the AOTMiT’s President) for your use.

The Health Policy Programme (HPP) template is specified by the regulation of the Minister of Health of 22 December 2017 on the health policy programme template, the template of the final report on the implementation of the health policy programme and the manner of preparation of the draft health policy programme and the final report on the implementation of the health policy programme (Journal of Laws 2017, item 2476).
Below, for your convenience, we provide a Health Policy Programme (HPP) template (wzór projektu PPZ) and a template of the final report (wzór raportu końcowego) in an editable version. We also hope that the information prepared by the Agency for Health Technology Assessment and Tariff System provides you with valuable and helpful tips for the creation of HPP.
At the same time, we would like to point out that the Agency is not authorised to issue interpretations of legal provisions, therefore any doubts regarding individual provisions of laws and regulations should be addressed to dedicated bodies.
